
Ravenscroft Homelink provides a number of specialist services into the community,
which are called “intermediate care services”. Intermediate care focuses on
an individuals needs with the purpose of avoiding an inappropriate placement
into a residential setting i.e. the aim is to keep people in their own homes
within their community....longer.
The three main strands of intermediate care are;
Crisis Support Services
Domiciliary Rehabilitation services
Extracare Supported Housing Services
Homelink provides crisis support services in partnership
with Social services and the Health Authorities to clients in their own homes.
This service is managed from our Head office in Whitestake by our Head of Care
services. A specific care plan is drawn up in relation to the needs of the
individual client and is delivered by a team of Key Support Workers supervised
by Co-ordinators on a continuous basis for upto 72 hours. This service is delivered
exclusively by Ravenscroft Homelink for the Preston and South Ribble districts.
Homelink provides highly effective domiciliary rehabilitation services in
partnership with Social Services and the Health Authority to clients in their
own homes. This service is also managed from our Head Office in Whitestake.
The clients receive an intensive re-enablement programme of rehabilitation
for upto 8 weeks via our team of Key Support Workers. This service is delivered
exclusively by Ravenscroft Homelink for the Chorley, West Lancashire, Preston
and South Ribble districts.
Homelink provides extracare supported housing services in partnership
with Social Services, the Health Authority and Housing associations to clients
in their own tenancies within sheltered housing schemes. The service is aimed
at ensuring that clients are able to maintain their own tenancies by providing
a more intensive homecare service. This service is provided exclusively by Ravenscroft
Homelink for the Preston and South Ribble districts. |
